Retatrutide Phase 3: What TRIUMPH-2 and TRIUMPH-3 Actually Reported
Retatrutide's Phase 2 data made it famous. The Phase 3 programme is what determines whether it becomes a medicine — and in 2026 the readouts kept arriving.
Why the triple agonist is different
Retatrutide activates three metabolic receptors at once:
- GLP-1 — satiety, insulin secretion, gastric emptying
- GIP — insulin response, adipose tissue handling
- Glucagon receptor — energy expenditure, hepatic fat metabolism
For comparison: semaglutide targets GLP-1; tirzepatide targets GLP-1 and GIP. The glucagon component is retatrutide's distinguishing feature, and the leading hypothesis for why its weight-loss numbers sit above its predecessors — it appears to raise energy expenditure rather than only reducing intake.
TRIUMPH-2: obesity with type 2 diabetes
1,152 adults with obesity or overweight plus type 2 diabetes, randomised to retatrutide 4mg, 9mg, 12mg or placebo, over 80 weeks.
Mean body-weight reduction from baseline:
- 4mg — 12.7% (≈29.8 lb)
- 9mg — 19.1% (≈45.4 lb)
- 12mg — 20.8% (≈49.6 lb)
- Placebo — 4.0% (≈9.3 lb)
That context matters: people with type 2 diabetes have historically lost weight more slowly than those without it on the same agents. Exceeding 20% in this population is the notable part, not the headline percentage in isolation.
Glycaemic control, from a baseline A1C of 7.7%:
- 4mg — −1.4%
- 9mg — −1.6%
- 12mg — −1.5%
- Placebo — −0.2%
TRIUMPH-3: severe obesity with cardiovascular disease
1,949 participants with BMI ≥35 and established cardiovascular disease, with or without type 2 diabetes, receiving 9mg, 12mg or placebo.
This is the harder population and, arguably, the more clinically meaningful one — cardiovascular outcomes are what regulators and payers weigh most heavily.
Across the programme
Combining the readouts reported to date:
- Up to 22.6% average body-weight reduction
- Up to 25.3 kg (55.8 lb) average weight loss
- Up to 1.6% A1C reduction
- Improvements reported across triglycerides, blood pressure, inflammatory markers and waist circumference
- Positive results across five completed Phase 3 studies
- Eli Lilly has stated it plans to submit a Biologics License Application to the FDA in Q1 2027
What the numbers don't say
- Gastrointestinal adverse events remain the dominant tolerability issue across the class
- Dose-dependent heart rate increases were observed in earlier trials and warrant continued attention
- Trial populations are selected; they are not a general population
- Weight regain after discontinuation has been the pattern across the incretin class and there is no reason yet to assume retatrutide is exempt
- Percentages are group means. Individual response distributions are wide.
The classification question
Retatrutide is being filed as a biologic, not a small molecule. That's a regulatory distinction with real downstream consequences for how it can be produced, compounded and accessed once approved — and it's the reason discussion of retatrutide availability has changed tone through 2026.
For researchers
The Phase 3 data narrow the uncertainty considerably on efficacy and shift the interesting questions elsewhere: durability after cessation, lean-mass preservation, the specific contribution of glucagon agonism to energy expenditure, and how triple agonism behaves in populations the trials excluded.
